Looking for crew from 9/7/1970 CH 46 crash

I am trying to locate the crew of CH 46 Bureau# 154793, which crashed and burned on September 7, 1970 while attempting to insert two patrol teams from Charlie Company, 1st Recon Bn in support of Operation Imperial Lake. The aircraft incident summary lists the pilot as LTCOL Gerald S. Pate, CO, HMM 262. The Hospital Corpsman assigned to one of the teams aboard the aircraft rescued at least one of the crew and/or members of the patrol teams and received a Meritorious Combat Promotion for his actions. He was also supposed to have been put in for a medal but never received it. We are trying to help him receive the recognition for his actions that he was promised in 1970.
